1. Give us a phone number that can receive text messages. We'll text you to confirm details and to request a photo of your cart — it's by far the fastest way to get your order moving.

2. Be ready to send one photo of your cart, taken from the front corner angle of the driver's side. That single angle shows the equipment we have to verify — headlights, turn signals, mirrors, windshield and wiper — without you having to send half a dozen pictures.

⚠️ Federal Warning — Photos Must Be Real and Unaltered

You are applying for an NHTSA-certified VIN and Manufacturer's Certificate of Origin for a road-legal motor vehicle. Your photo is the evidence we certify from. It is not a formality.

Submitting an AI-generated, edited, borrowed or otherwise altered image to show safety equipment your cart does not actually have is fraud against the federal government — not a dispute with us.

Depending on the charges brought, that carries 5 to 20 years in federal prison and substantial financial penalties. The statutes that apply include:

False statements to a federal agency — 18 U.S.C. § 1001: up to 5 years
Wire fraud — 18 U.S.C. § 1343, which sending the image electronically can constitute: up to 20 years
Federal VIN fraud & identity-document fraud — 18 U.S.C. § 511, § 1028 and related statutes: up to 5 years, and more where aggravated
State forgery and DMV fraud — charged separately under your state's vehicle and penal codes, in addition to any federal charge

We verify every photo, and we refuse orders. If your cart is missing equipment, tell us — we will help you source it. There is no penalty for not being ready yet. There is a very serious one for pretending you are.

Please note: Every conversion we document is required to have a windshield wiper — your documents are issued from New Hampshire, and New Hampshire requires one. Reflectors are required in every state under federal law (FMVSS 500). These are the two items most people miss or forget about. Don't worry about it — we can still process your order and simply add them for you. A complete LSV-compliant drill-less wiper kit is $69, and a complete reflector set — 4 orange and 4 red DOT reflectors — is $25. Once either has been added it cannot be removed — the order would have to be cancelled and placed again, and a $49 fee applies when reapplying after an incomplete order.

I understand and agree. If my cart does not already have an LSV-compliant windshield wiper or the required DOT reflectors, I authorize Unleashed EV to add them to my order at $69 and $25, and I agree to pay for them as part of my order total. I understand these items are required for my cart to be certified, that once added they cannot be removed, and that my order cannot be completed without them.

Certification & Waiver

Before we can issue your VIN package, you must certify the following. Read carefully, then check the box to agree.

By placing this order, I am certifying that my golf cart meets all Low-Speed Vehicle equipment standards required by both the State of New Hampshire (where Unleashed EV manufactures and federally titles the vehicle) and my own state, as well as FMVSS 500 — including headlights, tail/brake lights, turn signals, reflectors, parking brake, mirrors, windshield, windshield wiper, seat belts, DOT-approved tires, horn, VIN plate, and a top speed capability of 20–25 mph on a paved level surface. I understand it is not possible for Unleashed EV or GolfCartVIN.com to inspect my vehicle in person, and I am certifying that all required equipment is in place and meets standards. If my cart is gas-powered, I further certify that I have confirmed my state currently allows gas-powered Low-Speed Vehicles to be registered and operated on public roads, and I understand that my order is non-refundable if my state does not allow gas-powered LSVs — state rules on gas carts change frequently and are outside our control. I agree to hold Unleashed EV and GolfCartVIN.com harmless from any claim, damage, fine, or injury resulting from operation of my cart without the proper equipment in place or in violation of state or federal law.

⚠ Important — Please ReadPayment is due immediately upon receiving your invoice. Unpaid orders will be deleted at the end of the business day. You are welcome to reapply, but a $49 fee will be assessed for the previous incomplete order.

We understand things come up. If you find you need more time to pay the invoice, simply give us a call and we'll put your order on hold rather than being deleted.
