Every state DMV requires roughly the same documents to title and register a golf cart as a Low-Speed Vehicle (LSV). The specific forms vary, but the core paperwork is consistent across all 50 states.
Here's the complete checklist — and where each document comes from.
1. A 17-digit VIN
This is the document most owners are missing. Standard golf carts come with a manufacturer serial number stamped on the frame, but that's not a VIN. State DMVs only accept 17-digit Vehicle Identification Numbers issued by NHTSA-registered manufacturers.

What to do if you don't have one: You can't get one from your existing cart manufacturer (Club Car, EZ-GO, Yamaha) because they don't issue VINs to converted carts. Order one from us — $500, ships in 1–2 business days.
2. Manufacturer's Certificate of Origin (MCO)
The MCO is the original "birth certificate" of the vehicle. It's the primary document the DMV uses to issue your title. Without it, no title.
Where it comes from: The manufacturer that issued the VIN. We print yours on security paper and include it in your shipped package.
Notarization required in eight states: Arizona, Kentucky, Louisiana, Missouri, North Carolina, Ohio, Oklahoma, and Pennsylvania. We notarize it for free before shipping if you're in one of those.
3. FMVSS 500 DOT Compliance Label
This is a small metal or foil sticker that gets permanently affixed to your cart, certifying it meets Federal Motor Vehicle Safety Standard 500 (the LSV-specific federal safety standard). DMVs require this to be physically on the cart before they'll issue a title — most states will inspect for it.

4. Bill of Sale
A document showing the transfer of ownership from the seller to you. Federal law requires a Bill of Sale for any titled vehicle transaction.
Where it comes from: Your seller, or the manufacturer if it's a new title. We include a federally-acceptable Bill of Sale with your package showing Unleashed EV as the manufacturer-of-record.
5. Odometer Disclosure Statement
Required by federal law (Truth in Mileage Act) for any title transfer in all 50 states. It records the cart's mileage at the time of sale.
Where it comes from: Standard federal form — we include it in your package, pre-filled.
6. Tire and Loading Placard
Lists the cart's GVWR, GAWR, tire size, and PSI. Some state DMVs check for this physically on the cart at registration; others require a copy in your application.
Where it comes from: Federal-form placard included with your VIN package, pre-printed for your specific cart's specs.
7. Proof of insurance
Every state that allows LSVs on roads requires insurance. You'll need the insurance card or binder in hand at the DMV.
Where it comes from: Your insurance agent. Get an LSV policy or add LSV coverage to your existing auto policy. Premiums are typically $15–$35/month.
8. Valid driver's license
You must have a valid state driver's license to operate an LSV on public roads. Some states (like Florida) require the license to be from that state.
9. Your state's title application form
Each state has its own title application form (Florida has HSMV 82040, Texas has Form 130-U, etc.). Available at the DMV office or downloadable from your state DMV website.
Pro tip: Fill it out at home before going. The DMV moves much faster when you arrive prepared.
10. Payment for state fees
